How to File an Incident Report

To guide participants and leaders in filing Incident Reports that support safety improvements across Mountaineers activities, events, and lodge stays.

At The Mountaineers, safety is a shared responsibility—and incident reporting plays a vital role in helping us learn, adapt, and prevent future harm. We encourage everyone, whether a participant or a leader, to file an Incident Report anytime an accident, injury, or even a near miss occurs. Your report, no matter how minor the incident may seem, helps us foster a safer environment for all.

For Participants

If you experienced or witnessed an incident during a Mountaineers activity, lodge stay, or event, you can easily file a report:

Activities

  1. Go to your My Profile page.
  2. Under My Activities, find the activity in question.
  3. Scroll to the bottom of the activity’s detail page and click “Report an Incident.” on the right-hand side.

Tip: You can also find activities using the site-wide search if they don’t appear under My Activities.

Lodge Stays

  1. Visit My Lodge Stays on your My Profile page.

  2. Select the relevant lodge stay.

  3. Scroll down and click “Report an Incident.”

Events

  1. Go to the event’s Feedback Form (linked in your My Activities or Events).

  2. Answer “Yes” to the safety concern question.

  3. You will be automatically redirected to the Incident Report form.

Tip: This process also works for submitting feedback for Activities and Lodge Stays.

For Leaders

As a leader, you are strongly encouraged to report any incidents that happen during your activity, event, or lodge stay. Here's how:

  1. Navigate to the Roster of your activity, event, or lodge stay.

  2. Scroll to the bottom of the roster page.

  3. Click the “File an Incident Report...” button under the Report an Incident section.

  4. Complete the form with as much detail as possible.

Leaders may also encourage participants to file their own reports if they experienced or observed the incident firsthand.

Why Reporting Matters

Incident reports are reviewed by our Safety Committee to identify patterns, improve training, and refine our risk management practices. Even “near misses” offer valuable lessons—reporting them could help prevent a future injury or worse.

When in doubt, report it.
